How to report a lost or stolen passport, why you replace it in person with Form DS-11, what to bring and what the government charges, with official sources.

- Online, on the official State Department website.
- By mail with Form DS-64, if you are only reporting it for now. Include a copy of the front and back of your photo ID.
- With your new application, if you are applying for a new passport now: you report it together with Form DS-11 at the acceptance facility.

A lost or stolen passport is on that list, even if you could renew by mail before. Acceptance facilities are often post offices, libraries or local government offices. Find one near you with the official search: under review — see the official source6.

Step by step
- Report the passport as lost or stolen: online, by mail with Form DS-64, or at your appointment together with Form DS-11.
- If it was stolen and you filed a police report, keep a copy for your application.
- Fill in Form DS-11 on a computer and print it single-sided. Do not sign it yet.
- Write down, as clearly as you can, where and when you lost the passport or when it was stolen.
- Gather your original proof of U.S. citizenship, your photo ID, the photocopies and one passport photo.
- Find an acceptance facility near you and check how it receives applicants.
- Go in person, sign Form DS-11 when the agent asks you to, and pay the application fee and the acceptance fee separately.
- Watch your email for status updates and keep your receipt.

What to bring to the acceptance facility
- Form DS-11, printed single-sided and not signed
- Where and when the passport was lost or stolen, written down
- A copy of the police report, if you filed one
- Original proof of U.S. citizenship and a photocopy
- Photo ID and a front-and-back photocopy
- One passport photo, not stapled
- Check or money order for the application fee, payable to "U.S. Department of State"
- Separate payment for the acceptance fee
- Form DS-64, if you are reporting the loss at the same time
Do and don't
Do
- Report a valid passport as soon as you notice it is missing.
- Keep proof of your report and a copy of any police report.
- Keep copies of everything you hand in.
Don't
- Don't travel with a passport you reported, even if it turns up later.
- Don't report a passport that had already expired.
- Don't try to replace a lost or stolen passport with a mail renewal (Form DS-82).
- Don't sign Form DS-11 before the acceptance agent asks you to.

Frequently asked questions
I found my passport after I reported it. Can I use it?
No. Once you report a passport lost or stolen, it is cancelled and cannot be used for travel, even if you find it. Apply for a new one with Form DS-11.
My lost passport had already expired. Do I report it?
No. The State Department asks you not to report an expired passport. To get a new one, you still apply in person with Form DS-11, because your last passport was lost.
Do I need a police report?
Only if you filed one: then include a copy with Form DS-11. Either way, describe where and when the passport was lost or stolen, because missing details can pause your application.
I lost my passport abroad. What do I do?
