Where to mail your U.S. passport application (DS-82, DS-5504, DS-64)

The official mailing addresses for mail renewals, name changes and loss reports, why you never mail Form DS-11 yourself, and how to send your envelope safely.

This address is only for the report. It does not get you a new passport. To replace it, you apply in person with Form DS-11: see Lost or stolen passport.

Form DS-11: you do not mail it

You apply in person with Form DS-11 in any of these situations:

You take Form DS-11 to an acceptance facility, often a post office, library or local government office, and hand it in there. You do not put it in the mail yourself. under review — see the official source13 Find a facility at under review — see the official source14.

Step by step

  1. Confirm which form you are sending: DS-82, DS-5504 or DS-64. If it is DS-11, do not mail it.
  2. Choose routine or expedited service. For expedited service, write "EXPEDITE" in large letters on the outside of the envelope.
  3. Find the address for your form, your state and your service speed above, and copy it exactly, including the full ZIP code.
  4. Put everything in one envelope: the signed form, the photo, your passport and documents, and the check or money order.
  5. Mail it with the U.S. Postal Service, using a service with tracking.
  6. Keep a copy of everything you sent and your mailing receipt.
  7. Wait for the first status update, then follow your application (see Track your passport application).

Before you seal the envelope

  • The right form for my situation (DS-82, DS-5504 or DS-64)
  • The address matches my form, my state and my service speed
  • "EXPEDITE" written on the outside, if I pay for expedited service
  • Form signed and dated
  • Photo, passport, documents and payment inside, as the form asks
  • A copy of everything kept at home
  • Sent with the U.S. Postal Service, with tracking

Do and don't

Do

  • Copy the address exactly as it appears here, line by line.
  • Use an envelope big enough that you do not have to fold the photo.
  • Keep the tracking number until your passport and documents are back.

Don't

  • Don't mail Form DS-11 yourself; it is handed in at an acceptance facility.
  • Don't send a renewal to the Form DS-64 address, or a loss report to a renewal address.
  • Don't use an expedited address unless you are paying for expedited service.
  • Don't use these addresses if you live outside the United States and Canada.

The address for your form and service speed. If you pay for expedited service, also write "EXPEDITE" on the outside of the envelope.

How do I know my application arrived?
